Your safety and well-being are our top priorities at the Song & Stage Bootcamp. To ensure that we can provide the best possible care in case of an emergency and maintain a safe environment for all participants, we require that each student (and their parent/guardian, if under 18) complete this Medical and Liability Release Form.

This form provides our staff with the necessary information to respond to any medical situations that may arise and outlines your agreement to assume responsibility for potential risks associated with camp activities.

Consent for Medical Treatment *

I, the undersigned, authorize the Hughes Brothers Theatre, the Song & Stage Bootcamp, and Branson Academy staff and their authorized representatives to obtain medical treatment for the participant in the event of an emergency, illness, or injury. I understand that reasonable efforts will be made to contact me or my emergency contact prior to medical treatment whenever possible. I agree to assume financial responsibility for any medical treatment provided to the participant.

Liability Waiver and Release *

In consideration of the participant’s voluntary participation in the Song & Stage Bootcamp, I, the undersigned, on behalf of myself or my minor child, hereby release, waive, and discharge the Hughes Brothers Theatre, the Song & Stage Bootcamp, Branson Academy, their owners, staff, volunteers, and any affiliated parties from any and all liability, claims, demands, actions, or causes of action for personal injury, illness, property damage, or death arising out of or related to participation in the camp, including any activities conducted on or off the premises. I understand that participation in the Song & Stage Bootcamp involves activities such as singing, dancing, acting, and physical movement, which may carry inherent risks. I voluntarily assume these risks.

Permission to Transport *

I authorize the Hughes Brothers Theatre and Song & Stage Bootcamp staff to transport my child (if under 18) to and from camp-related activities when necessary.
